What is the difference between primary and secondary hypogonadism?

Primary hypogonadism occurs when the testes fail to produce adequate testosterone despite normal pituitary signaling, while secondary hypogonadism results from insufficient hormone signals from the pituitary or hypothalamus.

Hypogonadism divides into two clinical categories based on where the problem originates in the hormonal chain. In primary hypogonadism, the testes themselves are dysfunctional or damaged and cannot produce testosterone, even when the pituitary gland sends appropriate luteinizing hormone (LH) signals. Causes include Klinefelter syndrome, testicular trauma, prior steroid use, or age-related decline. Blood work shows low testosterone paired with high LH levels, as the body attempts to compensate for the failing testis.

Secondary hypogonadism stems from problems higher up the chain, in the pituitary gland or hypothalamus, which fail to send adequate signaling hormones to the testes. The testes themselves are healthy but receive insufficient stimulation. This pattern produces low testosterone alongside low or normal LH and follicle-stimulating hormone (FSH). Causes include obesity, metabolic syndrome, untreated sleep apnea, or tumors affecting the pituitary.

This distinction matters because it changes treatment strategy. Men with primary hypogonadism typically proceed directly to testosterone replacement therapy. Those with secondary hypogonadism may first address underlying conditions like sleep issues or weight, and some candidates qualify for fertility-preserving medications that stimulate the pituitary rather than bypass it. Proper diagnosis through hormone testing guides TRT providers toward the appropriate clinical path.
